Our experienced Service Planners develop an understanding of each consumer throughout the assessment & ongoing service provision process. The information gathered not only accounts for their physical and general care needs, but serves to develop a story around who they are spiritually, socially and culturally. This dialogue supports our understanding of individual care recipients circumstances encouraging a holistic, strengths based approached centred around the person determining:

  • what & who are important to the person
  • what can the person do for themselves utilising an enablement framework
  • what additional supports can be accessed to promote a care recipients independence and social connections

WHAT TYPE OF SERVICES ARE PROVIDED?

The services provided will depend on the client’s assessed clinical care needs and include:
