AHome Care package (HCP) provides a co-ordinated package of services tailored to meet your specific care needs to;

  • Help you stay in your own home and be connected to your local community.
  • Give you choice and flexibility in the way your support services are delivered.
There are 4 levels of care through Home Care packages ranging from Level 1 (low level) to Level 4 (high level )

Under a Home Care package we can provide a range of support services but not limited to:

  • Personal Care – such as help with showering, dressing and mobility
  • Home Help and Maintenance - support with domestic activities, including meal preparation, garden and general home maintenance
  • Social Outings – that keep you connected with family and friends and your community such as bus trips, cafes, community events and shopping
  • Respite to give your carer time out
  • Transport to appointments with your GP or allied health
  • Access to Alternative Therapies such as Reflexology, Massage etc
  • Clinical Nursing by Registered Nurses
  • Supply of Equipment and Aids
  • Lifestyle Activities to improve your quality of life
  • Access to Allied health services for example physiotherapy, podiatry, occupational therapy etc
  • Access to Cultural activities
  • Care co-ordination and case management

We ensure that we work closely with the consumer and their family towards their health & wellness goals whilst ensuring the support received suits the person’s lifestyle choices and daily routines.

ELIGIBILITY for home care packages

My Aged Care is the first call for all your aged care needs. My Aged Care can give you information on what services and supports are available to you or the person you care for.

A My Aged Care representative can also discuss eligibility for services and explain the costs of aged care services.

My Aged Care has a website www.myagedcare.gov.au that provides the information you need about aged care and the assessment process.

Call the contact centre between 8am and 8pm Monday to Friday and 10am to 2pm on Saturdays (local time) by calling 1800 200 422.

The Translating and Interpreting Service (TIS) on 131 450 covers more than 100 languages and is available 24 hours a day, seven days a week.

CONSUMER DIRECTED CARE

The Consumer Directed Care approach (CDC) puts you – the consumer – in the driver’s seat if you wish. You choose the level of involvement you wish to have from service planning and budgeting yourself or with the assistance of a carer or family member or whether we offer you those services as part of your packaged care.


My Aged Care is the entry point for accessing aged care services. A Member of the Aged Care Assessment Team (ACAT) will visit you in the home to carry out an assessment to determine what level of care is most suitable to you and your situation.

CHANGES FROM FEBRUARY 2017

From February 2017, funding for Home Care Packages will follow the consumer. This means they are able to select the provider of their choice according to their needs and preferences. Home Care Packages will therefore be portable. Giving increased power to the consumer as they can change providers according to their needs. These changes will see a less regulated, more consumer-driven and market based aged care system.

Our wellness and reablement approach to service delivery aims to promote independence and autonomy.

Our Service Planners will listen to what’s important to you and develop a Support Plan in consultation with you, your family or carer. You determine your level of involvement in how your home care package is used, so that you can obtain the supports and lifestyle you want.

We are competitive with our Administrative and Case Management charges ensuring your package of care and support is not taken up with excessive costs in order to maximise the available funds to support your needs.
