NSW Home Support Services has been delivering In-home nursing services to eligible veterans since 1991. Since then we have had the privilege of providing DVA community nursing services to hundreds of veterans, war widows and war widowers on the Mid North Coast assisting them to maintain their independence and providing them with the choice of being cared for at home.

The DVA Community Nursing Program is providing the veteran home care community with access to community nursing services to meet their assessed clinical and personal care needs delivered in their homes.

WHAT IS THE COMMUNITY NURSING PROGRAM DVA?

The DVA Community Nursing program provides eligible members of the veteran community with access to community nursing services to meet their assessed clinical and personal care needs delivered in their homes.

  The Community Nursing program’s aim is to enhance the independence and health outcomes of the entitled person by avoiding early admission to hospital and/or residential care by providing access to community nursing services. The program is funded by the Australian Commonwealth government through the Department of Veterans Affairs. There is no cost to program recipients.

WHO CAN ACCESS DVA community nursing services?

Veterans and war widows or widowers who have a Gold Card are eligible for DVA Community Nursing. White Card holders may also be eligible for this service where the condition requiring nursing care is accepted by DVA as war related.

WHAT TYPE OF SERVICES ARE PROVIDED

The services provided will depend on the client’s assessed clinical care needs and include:

  • Assessment and Service Planning.
  • Personal Care.
  • Wound Care.
  • Medication management.
  • Incontinence management.
  • Palliative Care ( including care at home till end of life).
  • Referrals to other Allied Health or other community care services as needed.
  • Dementia Care.
  • Rehabilitation.
  • Chronic disease management.
  • Advocacy.
  • Assistance to access long term care arrangements.

What level of assistance is available?

The duration and frequency of visits required to a veteran will be determined upon the assessment of the client’s clinical needs.
